A complete enrollment packet and copies of the following documents must be provided to the school before students can be placed on an admissions list:
Birth certificate
Immunization record (shot record)
Unofficial or Official transcript
Documentation which describes custody arrangements, if parents are divorced.
Documentation, if a guardian is involved
Official Withdrawal from previous school
Title I survey form
AIMS and Terra Nova test results
If the student is a minor (under age 18), the parent or legal guardian must sign the registration application.
If the student has attained their majority (age 18), the student signs the registration application.
